WHEREAS, Dr. Alexander received her undergraduate and Veterinariae Medicinae Doctoris degree from the University of Pennsylvania; and
WHEREAS, while at the University of Pennsylvania, Dr. Alexander was the winner of the New Bolton Esquire Award (1982), was published in the Journal of Biochemistry (1978) and Journal of Animal Behavior (1982); and
WHEREAS, Dr. Alexander helped catapult a historic decrease in animal euthanasia while previously working at Chicago Animal Care and Control from 1989 to 1999; and
WHEREAS, Dr. Alexander, a true advocate who dedicated her life’s work in the caring of all animals, began her Cook County career in 2007; and

WHEREAS, IDOT requires a resolution to reaffirm the Forest Preserves funding commitment for Phase III Engineering (construction management) and Construction costs associated with the Project as per the Local Agency Agreement; and
WHEREAS, the Local Agency Agreement also provides for the division of Phase III and Construction costs for the Project and payment of 20% of the engineering and construction costs by the Forest Preserves as the Local Agency; and

Reason: IDOT requests temporary easements for regrading of existing berms along Golf Road to reduce flooding on the roadway near the CN Railroad underpass. The berms were originally constructed in 1930 and have eroded and/or settled over time. The berms are proposed to be raised to their original elevations.
The term of the temporary easement is five (5) years, or completion of construction, whichever comes first.
The existing berms are the only structures proposed to be rebuilt on Forest Preserves’ land.
The area for the temporary easement is 0.208 acre. IDOT indicates that thirty-two (32) trees are expected to be removed for this project.

Request: Authorization for the Forest Preserves of Cook County (the “Forest Preserves”) to enter into a supply contract with KLF Enterprises, Inc., Markham, Illinois
Reason: Project #17-31-126 (contract #74000004198) consists of the supply and delivery of sand, gravel, stone, screenings, and related granular materials for the repair and upkeep of trails, groves, parking lots, and facilities throughout the Forest Preserves. Bids were requested at a per ton rate for a variety of fine and coarse aggregate materials as well as stone riprap. The contract was competitively bid, with a total of two (2) vendors responding to the solicitation for bids.
KLF Enterprises, Inc. was the lowest bidder and offered a complete bid for all requested materials. Additionally, they demonstrated experience servicing large volume contracts with multiple delivery locations. This contract will be utilized by several Forest Preserves departments to consolidate and centralize districtwide procurement of granular materials.

WHEREAS, the accident occurred in Elk Grove Village on Arlington Heights Road, the employee was twenty years old under the influence driving a Ford F350 Crew dump truck with four individuals in the vehicle; and
WHEREAS, the employee has been terminated by the Cook County Forest Preserves and charged with reckless homicide and aggravated driving under the influence of drugs involving a death; and
WHEREAS, Elk Grove Police are leading the investigation, the investigation is ongoing; and
WHEREAS, driving under the influence is reckless, it needlessly puts the driver and other lives at risk; and

PROPOSED LICENSE REQUEST
Request: Authorization for the Forest Preserves of Cook County (the “Forest Preserves”), as Licensor to grant a License to Dan Ryan Woods Gold Star Mothers Memorial, Grantee, to relocate, revamp, and maintain a previously existing monument at Dan Ryan Woods. The Forest Preserves will construct a new foundation for the reconstructed monument. The original monument was located at the southeast corner of 87th Street and Western Avenue in the 1920s and was decommissioned and stored by the Forest Preserves in November 2017. The new location for the memorial will be in proximity to the Dan Ryan Visitor Center. Stones from the original monument will be used in the revamped memorial and the Grantee will replace the missing original main plaque with a replica.
