PROPOSED LICENSE REQUEST
Request: Requesting authorization for the Forest Preserves of Cook County (the “Forest Preserves”), as Licensor to grant a License to Zayo Group, LLC (“Zayo”), Grantee, to locate, operate and maintain 11,260 feet of buried fiber optic cable along Milwaukee Avenue, Willow Road and Sanders Road in Dam No. 1 Woods. This new license request will bring into compliance existing fiber optic cable infrastructure, which was granted in 2000 as license #1674. Since granting the original license, the owner of the fiber optic cable has changed to a different corporation and this new license will reflect the current owner of the infrastructure. Given that this is an administrative change and extension of a prior license, there are no tree impacts as part of this new license.
Term: Ten (10) Years (retroactive beginning 12/14/2020 - 12/13/2030)
License Fee: $348,966.94
License Application Fee: $1,500.00
License Review Fees: $3,000.00
YELAR Fee: $6,750.00
Total one-time upfront fee of $360,216.94 for the term of the License

Grantee has met the insurance requirements under the License Agreement.
The Forest Preserves staff have reviewed this license application and plans and have found them to be acceptable; therefore, it is recommended that the Forest Preserves of Cook County Board of Commissioners approve the issuance of this license.
