PROPOSED RESOLUTION

 

TO APPROVE AND ADOPT IDENTITY PROTECTION POLICY

 

WHEREAS, due to issues pertaining to identity theft, the Illinois General Assembly passed the Identity Protection Act (5 ILCS 179/1 et seq.) in 2010;

 

WHEREAS, the Forest Preserve District of Cook County (the “Forest Preserve”) recognizes that it is important to safeguard social security numbers against unauthorized access and has approved an Identity Protection Policy to comply with the Identity Protection Act;

 

WHEREAS, the Forest Preserve’s Identity Protection Policy does the following: (1) requires that all employees identified as having access to social security numbers in the course of performing their duties be trained to protect the confidentiality of social security numbers; (2) directs that only employees who are required to use or handle information or documents that contain social security numbers have access to such information or documents; (3) requires that social security numbers requested from an individual be provided in a manner that makes the social security number easily redacted if required to be released as part of a public records request; and (4) requires that, when collecting a social security number or upon request by the individual, a statement of the purpose or purposes for which the agency is collecting and using the social security number be provided.

 

NOW THEREFORE BE IT RESOLVED, by the President and the Board of Commissioners of the Forest Preserve District of Cook County that the Identity Protection Policy is hereby adopted by resolution effective immediately.
