Good(s) or Service(s): The Forest Preserve District of Cook County ("the Preserves") wishes to utilize CorVel Healthcare Corporation ("CorVel") for worker's compensation managed care, preferred provider, and claims administration services. These services are similar to those already being provided by CorVel to Cook County.
Contract Value: $525,000.00
Contract period: 11/19/2014 and continue until 9/30/2017 or until Agreement is terminated in accordance with its terms, whichever occurs first.
Estimated Fiscal Impact: FY 2014 $25,000.00, FY 2015 $175,000.00, FY 2016 $175,000.00 and
FY 2017 $150,000.00

Summary: On 9/10/2014, the Cook County Board approved contract #1318-13179 with CorVel following a Request for Proposals for Worker's Compensation Managed Care Claims Administration. The District is seeking to procure the same services provided by CorVel to the County pursuant to Section 1-8-2(X) of the District Code.
The District's contract will incorporate the same terms as the Cook County's existing contract.
